COMPARATIVE ANALYSIS OF SURGICAL OUTCOMES IN TOTAL LAPAROSCOPIC HYSTERECTOMY WITH UTERINE ARTERIES LIGATED BEFORE AND AFTER CORNUAL PEDICLE: A STUDY IN GYNECOLOGIC LAPAROSCOPIC SURGERY

TLH patients with both uterine arteries ligated before the cornual pedicle and TPHL patients with TPHL done after the cornual pedicle were compared with TPHL patients with both uterine arteries ligated before the cornual pedicle. The study was conducted at a gynecologic laparoscopic center on two groups of 104 women having TLH. Uterine arteries in group A are tied following the cornual pedicles. A harmonic scalpel or bipolar diathermy was used to desiccate each pedicle of group B before TLH. Uterus with cervix were removed via vasectomy or morcellation. Both groups of patients had dysfunctional uterine bleeding and myomas, requiring TLH. According to the study, group A's surgery lasted 73 minutes on average, while group B's lasted 63 minutes (P < 0.001). The blood loss in group A was 70 mL, while the blood loss in group B was 43 mL (P value< 0.001). Complications were not reported in either group. The procedure can be performed more quickly and with less blood loss if the uterine arteries have been ligated at their origins beforehand, and there are fewer complications if the vessels have been ligated at their origins before the surgeryddd
